AIFA has published the new OsMed Report 2025, which provides an updated overview of the use of medicinal products in Italy and the main trends in pharmaceutical expenditure.
Among the aspects that emerge most clearly:
💊 Pharmaceutical expenditure continues to rise, reaching €39.3 billion (+6% compared to 2024), of which more than €28 billion is borne by the National Health Service (SSN).
📈 Among the medicinal products with the greatest impact on pharmaceutical expenditure, PD‑1/PD‑L1 inhibitors and interleukin inhibitors remain in first place, both categories showing growth compared to 2024 (+9.1% and +2.8% vs 2024).
💉 In 2025, a decline was recorded in the consumption of systemic antibiotics in community care (–7.9% in DDD and –8.6% in expenditure), alongside a slight increase in purchases by public healthcare facilities (+2.4%).
🧒 In the pediatric population, a reduction in the use of antibiotics is observed, while the use of psychotropic medications continues to increase.
✨ After years of growth, in 2025 expenditure for innovative indications covered by the dedicated Fund recorded a 19.4% decrease, settling at €700 million — a reduction mainly linked to the expiry of the innovation period for certain molecules and the entry of new medicines with conditional innovativeness.
🧬 Expenditure on advanced therapies (ATMPs) continues to grow, reaching €222.6 million in 2025 (+14.5% compared to the previous year). Spending on orphan drugs is also increasing (+7.9%), amounting to €2.55 billion.
🌍In comparison with other European countries, Italy records pharmaceutical expenditure of €750 per capita, lower than Germany and Austria but higher than several other countries analysed. Italy also stands out for its leading position in the use of biosimilars, which account for 70.3% of expenditure and 71.4% of consumption, while the share of generic medicines remains more limited than in other countries.
📊 The Report confirms the role of demographic factors, therapeutic innovation, and prescribing patterns in shaping the evolution of pharmaceutical expenditure, highlighting the importance of data in guiding medicine‑related policies.
